Your first visit to Southwestern PA Endodontics establishes a vital foundation for our relationship with you. During the first visit, we make
sure to obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment. Our patient forms can be filled out online with your phone, tablet or computer any time prior to your appointment. Filling out the forms online before your next appointment will save you time in the office. If you need assistance in filling out your forms, we can help! Please call us ahead of time to request additional time so that we can assist you in completing your forms in-office.
To further assist you, please bring a list of your medications if you take 4 or more different medications so that we can quickly scan them into our system.
